Abstract

|
Hemophilic patients with a high-titre inhibitor to factor VIII show more severe arthropathy and clinical symptoms than other hemophilic patients without the antibody to factor VIII. However, orthopedic surgery in patients with a high-titre inhibitor to factor VIII has been delayed or withdrawn due to the limited efficacy of this factor. The recent development of recombinant factor VIIa (rFVIIa; NovoSevenⓇ), and its demonstrated safety and efficacy in treating inhibitor patients, has opened new possibilities for the treatment of these inhibitor patients. We experienced bilateral total hip arthroplasties using rFVIIa for bleeding control in one hemophilic patient who has factor VIII deficiency with high titer antibody. This case demonstrated a controlled rFVIIa injection combined with continuous infusion is very effective and safe in bleeding control during and after complicated and extensive surgery without complications. The continuous infusion of rFVIIa can be considered a feasible, convenient, safe and cost-effective alternative to intermittent bolus injections.
